Support Coordinator
We are seeking a highly skilled and compassionate individual to join our team as a Support Coordinator. This role will involve working closely with stroke survivors and their families to provide tailored support and guidance.
Key Responsibilities:
- Develop and implement personalized support plans to meet the unique needs of each individual
- Provide emotional support and guidance to stroke survivors and their families
- Collaborate with healthcare professionals to ensure comprehensive care
- Conduct regular assessments to monitor progress and adjust support plans as needed
Requirements:
- Excellent interpersonal and organizational skills
- Ability to work independently and as part of a team
- Strong communication and problem-solving skills
- Experience working with vulnerable populations
