Support Coordinator

We are seeking an innovative, passionate, and professional individual to join our Stroke Recovery Service as a Support Coordinator.

This is an exciting opportunity to work with stroke survivors and their families to provide support and enablement following stroke.

Key Responsibilities:

  • Provide support and guidance to stroke survivors and their families
  • Work collaboratively with the Stroke Recovery Service team to deliver high-quality support
  • Develop and maintain strong relationships with stakeholders, including healthcare professionals and community groups
  • Contribute to the development and implementation of service plans to meet the needs of stroke survivors

Requirements:

  • Excellent interpersonal and organisational skills
  • Ability to work independently and as part of a team
  • Strong communication and problem-solving skills
  • Experience working with vulnerable populations, preferably in a healthcare or social care setting
