We are seeking a highly motivated and experienced Physiotherapist to join our team as a Band 6 Children's Community Physiotherapist. The successful candidate will be responsible for leading the delivery of high-quality Physiotherapy interventions to patients in their own homes, preventing avoidable admissions to acute care and supporting discharges.
Key Responsibilities- Lead the development and implementation of Physiotherapy/Occupational therapy interventions for patients with complex needs, working closely with the Urgent Community Response Team and other agencies.
- Conduct comprehensive assessments of patients, including those with complex presentations, and develop individualized treatment plans to achieve quality of life and independence.
- Establish and maintain therapeutic relationships with patients and carers, using health coaching skills to engage and motivate them to participate in their treatment plans.
- Liaise with GPs, Social Services, inpatient teams, and other healthcare professionals to ensure identified needs are met and care is coordinated effectively.
- Participate in multi-disciplinary meetings and contribute to the development of care pathways for patients with complex needs.
- Ensure accurate and timely recording of activity data and clinical information on Systm One.
- Diploma or 1st level degree in Physiotherapy/Occupational therapy.
- Current HCPC registration.
- Clinical educators course.
- Experience of working at Band 6 level and evidence of good observational and reporting skills.
- Ability to work flexibly and manage stressful situations.
