About the Role

We are seeking a highly motivated and experienced Physiotherapist to join our team as a Band 6 Children's Community Physiotherapist. The successful candidate will be responsible for leading the delivery of high-quality Physiotherapy interventions to patients in their own homes, preventing avoidable admissions to acute care and supporting discharges.

Key Responsibilities
  1. Lead the development and implementation of Physiotherapy/Occupational therapy interventions for patients with complex needs, working closely with the Urgent Community Response Team and other agencies.
  2. Conduct comprehensive assessments of patients, including those with complex presentations, and develop individualized treatment plans to achieve quality of life and independence.
  3. Establish and maintain therapeutic relationships with patients and carers, using health coaching skills to engage and motivate them to participate in their treatment plans.
  4. Liaise with GPs, Social Services, inpatient teams, and other healthcare professionals to ensure identified needs are met and care is coordinated effectively.
  5. Participate in multi-disciplinary meetings and contribute to the development of care pathways for patients with complex needs.
  6. Ensure accurate and timely recording of activity data and clinical information on Systm One.
Requirements
  • Diploma or 1st level degree in Physiotherapy/Occupational therapy.
  • Current HCPC registration.
  • Clinical educators course.
  • Experience of working at Band 6 level and evidence of good observational and reporting skills.
  • Ability to work flexibly and manage stressful situations.
