An exciting opportunity has arisen to recruit a Band Generic Health Care Support Worker within the multi-disciplinary Community Resource Team based in North Wales. The Community Resource Team ethos is to focus on supporting patients to remain independent in their own homes with an individualised package of care to meet their needs.
Main Duties of the JobThe Health Care Assistant role will include assisting patients who need care and support with activities of daily living such as washing and dressing, meal preparation and prompting with the administration of medication. The role involves working in partnership with the dynamic multidisciplinary team and external agencies to meet individuals' needs.
The post holder will function as part of the Integrated Multidisciplinary Community Health Care team. He/she will be responsible for carrying out designated work, much of which will involve direct patient care. This care will be delegated by trained staff and the post-holder will be expected to work under the indirect supervision of trained staff.
