Join Our Team as a Clinical Coordinator

We are seeking a highly skilled and compassionate Clinical Coordinator to join our team at DaVita. As a Clinical Coordinator, you will play a vital role in delivering exceptional patient care and services in our outpatient dialysis center.

About the Role

As a Clinical Coordinator, you will be responsible for coordinating patient care and services, including trend lab values, tracking metrics on vital signs, weight, and outcome management. You will work closely with our clinical team to ensure the highest level of care is delivered to our patients, prioritizing their safety, comfort, and well-being.

Responsibilities
  • Coordinate patient care and services, including trend lab values and tracking metrics on vital signs, weight, and outcome management.
  • Work closely with the clinical team to ensure the highest level of care is delivered to patients.
  • Develop and implement patient care plans in collaboration with the healthcare team.
  • Monitor and report on patient outcomes and quality metrics.
  • Collaborate with the nursing team to ensure seamless patient care transitions.
Requirements
  • Current Registered Nurse (RN) license in the state of practice.
  • At least 18 months of registered nursing experience.
  • Dialysis Charge RN readiness approval by Clinical Nurse Manager or RN-licensed FA/GFA required.
  • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) preferred.
  • Intensive Care Unit (ICU), Critical Care Unit (CCU), Emergency Room (ER), or Medical Surgical (Med Surg) experience is preferred.
