About the Role
We are seeking a highly skilled Clinical Pharmacist to join our team at HealthBridge Direct, the Redbridge GP Federation. As a Clinical Pharmacist, you will play a crucial role in supporting the delivery of the Network Contract DES and Quality Outcomes Framework (QOF) in our general practices.
Key Responsibilities
- Provide clinical leadership on medicines optimisation and quality improvement
- Manage aspects of medicines safety and antimicrobial stewardship
- Work as part of a multidisciplinary team in a patient-facing role
- Take responsibility for areas of chronic disease management within the practice
- Undertake clinical medication reviews to proactively manage patients with complex polypharmacy
Person Specification
- Masters degree in pharmacy (MPharm) or equivalent
- Registration with General Pharmaceutical Council
- Post-graduate clinical diploma level or equivalent training/experience or independent prescriber
- A member of or working towards Faculty membership of the Royal Pharmaceutical Society
