We are seeking a highly motivated and experienced Consultant Physician in Elderly Care to join our dynamic team at Medway NHS Foundation Trust. As a key member of our Elderly Care department, you will play a vital role in delivering high-quality patient care and contributing to the development of innovative pathways for frail elderly individuals.
Key Responsibilities- Manage patients admitted to the Elderly Care ward, including those requiring acute frailty assessment and management.
- Participate in the Acute Frailty On-call service, working closely with other consultants to ensure seamless patient care.
- Develop and implement new models of care for frail elderly individuals in the community, in collaboration with existing community teams.
- Contribute to the teaching, research, and governance programs within the department, including participation in international conferences.
- MRCP or equivalent qualification.
- An appropriate higher degree (MD, PhD, or equivalent).
- GMC with a Licence to practice.
- On Specialist Register for Geriatrics/General Medicine or applying for CESR (Certificate of Eligibility for Specialist Registration).
- UK-based experience in Geriatrics at middle-grade level or above.
- CCT or equivalent in Elderly Care and General Medicine or within 6 months of CCT date at time of interview.
- A supportive and forward-looking department with opportunities for leadership and professional development.
- A negotiable subspecialty interest with all major choices, including community Geriatrics, Surgical Liaison for frailty, Falls, and more.
- Exciting new developments, such as the Emerald Frailty Unit, with opportunities to design innovative pathways for Acute Frailty assessment and management.
- A sustainable Frailty on-call model focused on Frailty, using the Clinical Frailty Scale to identify and select moderate to severe frailty presenting acutely.
