About the Role

We are seeking a highly skilled and compassionate Care Coordinator to join our team at NHS Professionals. As a Care Coordinator, you will play a vital role in ensuring that individuals receive the appropriate support and services to enhance their well-being.

Key Responsibilities
  • Assess clients' care needs and develop personalized care plans
  • Coordinate with healthcare professionals, social workers, and family members to provide comprehensive care
  • Monitor and evaluate the effectiveness of care plans
  • Schedule appointments and arrange transportation for clients
  • Maintain accurate records of client interactions and progress
  • Provide emotional support and guidance to clients and their families
What You'll Learn
  • An in-depth understanding of the roles and responsibilities involved in working within the NHS
  • Knowledge of the systems used to effectively complete your role to the highest standard
  • A sense of teamwork, gained through working alongside and supporting colleagues from all levels within the organization
Requirements
  • Educated to NVQ 3 Level or equivalent in a relevant subject, such as Administration, IT, or Health/Social Care
  • Experience working in a busy, challenging environment
  • Intermediate knowledge of IT systems, databases, and software programs, such as Outlook, Word, Excel, and PowerPoint
  • Clear communication skills, with excellent verbal, writing, data entry, and presentation skills
