About the Role
The Northern Care Alliance NHS Foundation Trust is seeking an experienced and skilled Advanced Practitioner to join our Bevan Intermediate Care Unit team. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality patient care and contributing to the development of our services.
Key Responsibilities
- Undertake a significant clinical caseload to an advanced professional standard, utilizing specialist knowledge and expertise.
- Develop an extended scope of practice beyond your primary profession, working collaboratively with healthcare colleagues.
- Triage referrals, either independently or with medical colleagues, and make onward referrals as necessary.
- Support the redesign of patient referral flows from primary care to secondary care, ensuring seamless transitions and optimal patient outcomes.

Requirements
- Current Professional Registration with NMC, HCPC
- GCSE English and maths to grade C
- Non-medical prescribing
