About the Role

We are seeking a highly skilled and experienced Mental Health Practitioner to join our team at Sussex Partnership NHS Foundation Trust. As a Clinical Link Practitioner, you will play a vital role in bridging the gap between primary and secondary care services, providing expert support and guidance to GPs and primary care teams.

Key Responsibilities
  1. Clinical Link Work
  • Act as a link clinician to primary care teams, providing clinical expertise and support to GPs and primary care professionals.
  • Assess and manage patients with mental health problems, providing evidence-based interventions and advice to GPs and primary care teams.
Collaboration and Partnership Working
  • Work closely with GPs and primary care teams to develop care plans for patients, ensuring seamless transitions between primary and secondary care services.
  • Provide training and education to GPs and primary care professionals on mental health issues, promoting a recovery-focused approach.
Service Development and Improvement
  • Contribute to the development of mental health services, identifying areas for improvement and implementing changes to enhance patient care.
  • Work closely with secondary care services to ensure effective communication and collaboration.
Requirements
  • Registered Mental Health Nurse, Social Worker, or Occupational Therapist with a strong background in mental health.
  • Excellent clinical skills and experience in assessing and managing patients with mental health problems.
  • Strong communication and interpersonal skills, with the ability to work effectively with GPs and primary care teams.
  • Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
