Main Responsibilities
- Conduct health assessments, administer medication, and perform diagnostic tests to ensure accurate diagnoses and effective treatment plans.
- Provide and evaluate health interventions to meet patients' health and social care needs, including treatment, preventative care, and patient education.
- Administer childhood immunizations and travel vaccinations, and manage patients with long-term health conditions.
- Assess, diagnose, plan, implement, and evaluate treatment plans for patients presenting with undifferentiated diagnoses.
- Proactively identify, diagnose, and manage treatment plans for patients at risk of developing long-term conditions.
- Diagnose and manage acute and chronic conditions, integrating drug- and non-drug-based treatment methods into management plans.
- Work with patients to support compliance with and adherence to prescribed treatments, and provide information and advice on medication, medication regimes, side effects, and interactions.
- Support patients in adopting health promotion strategies that promote healthy lifestyles and self-care.
About Us
Primary Care Careers is a rural General Medical Services (GMS) 9900 patient surgery with 4 Partners, 4 Salaried GPs, and a fantastic clinical and practice team. We are located in a modern, recently extended surgery with community nurses on site and an X-ray suite.
Person Specification
Essential Qualifications
- A relevant nursing degree.
- Current NMC registration with an active PIN.
- Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications.
- A registered prescriber.
- Competence to undertake asthma, COPD, and diabetic clinics and reviews, holding relevant certificates.
Essential Experience
- Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations, and immunizations, assisting with minor surgery, dressing, and wound care, and recording spirometry.
- A compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work.
- Knowledge and experience of supporting long-term conditions.
- Insight and understanding of current issues in nursing and the wider NHS.
