We are seeking a skilled and compassionate Care Coordinator to join our team at Home Meadow in Toft Cambridge. As a Care Coordinator, you will play a vital role in ensuring the wellbeing and social enhancement of our residents.
Key Responsibilities- Develop and implement activity programmes that cater to the diverse needs and interests of our residents.
- Collaborate with the local community to establish links and encourage social interaction.
- Work closely with the care team to ensure a holistic approach to resident care.
- Monitor and evaluate the effectiveness of activity programmes and make recommendations for improvement.
