About the Role

We are seeking a skilled and detail-oriented individual to join our Somerset Hub for Coordinating Care team as a Patient Transfer Facilitator. In this role, you will play a crucial part in facilitating smooth referrals for service users and patients, providing a single point of contact for information on patients to be transferred or admitted to community hospital beds within the Primary Care Trust.

Main Responsibilities

As a Patient Transfer Facilitator, you will work closely with our clinicians to provide an efficient referral process, including booking transport and liaising with patients. You will also be responsible for coordinating telephone calls, completing reports, and working across multiple computer systems.

Requirements

  • Excellent written and verbal communication skills
  • Experience in the use of IT systems
  • ECDL or ability to use Microsoft Word, Excel, and Outlook
