Eligibility Requirements

Key Responsibilities
Following a successful re-tender, we are enhancing the West Midlands Health in Justice profile to establish a distinctive Regional Specialist Clinical Hub.
Under the guidance of the Regional Primary Care Lead and Deputy Regional Manager, the team will comprise skilled Advanced Clinical Practitioners and Advanced Clinical Practice Nurses.
Based at the West Midlands Regional Office, the team will operate independently, initially covering all three Wolverhampton sites, with plans for future expansion throughout the West Midlands.
Advanced Clinical Practice Nurses will concentrate on prevalent local health issues, providing specialized insights into long-term conditions such as obesity, hypertension, asthma, and diabetes. Responsibilities include:
- Enhancing clinical areas like health promotion, chronic disease management, and preventive health services
- Supporting the regional team in reviewing clinical policies and procedures
- Contributing to the national care planning initiative

Qualifications
Essential
- Registered Nurse: Adult Nursing with current NMC registration
- Postgraduate qualification or specialist interest in obesity, diabetes, hypertension, asthma, or frailty
- Experience working autonomously in a primary care setting, adhering to best practice guidelines and effectively assessing patient risks
- Understanding of clinical audit and its connection to research
