This role is responsible for providing support to vulnerable young people and children, including those who are refugees or unaccompanied minors. The successful candidate will work closely with a multi-disciplinary team to assess and meet the needs of individuals, relatives, and carers.
Key Responsibilities- Conduct assessments and develop care plans to meet the needs of vulnerable young people and children.
- Work in partnership with users, carers, relatives, and other agencies to identify current and future service requirements.
- Decide on the most appropriate level and type of service provision to meet identified needs and risks.
- Manage day-to-day case management activities, including arranging and conducting meetings.
To be successful in this role, you will need to be a qualified and registered Social Worker with experience in working with children and families, including those from vulnerable groups. You will also need to be highly skilled at building meaningful relationships with children and families to engage them in assessments and plans.
Additional InformationThis role requires the ability to travel to various locations within Hampshire and the Isle of Wight, and a current driver's license and access to a reliable vehicle are essential.
