Job Summary

We are seeking an experienced and skilled nurse to join our Older Peoples Mental Health Intensive Home Treatment Team as a full-time band 6 nurse. The successful candidate will work in a dynamic and supportive environment, providing high-quality care and treatment to patients in their own homes.

Main Responsibilities
  • Triage referrals and conduct comprehensive mental health assessments in patients' homes or care facilities.
  • Develop and implement effective care plans, working closely with the multidisciplinary team and consultant psychiatrist.
  • Provide education and support to patients and their families, promoting recovery and well-being.
  • Collaborate with other healthcare professionals, social care agencies, and community services to ensure seamless care coordination.
  • Administer and monitor medication in the community, ensuring patient safety and adherence to treatment plans.
  • Prevent unnecessary hospital admissions by providing intensive support at home, promoting early discharge and leave pathways.
  • Engage in regular zoning meetings with the team and managers to discuss patient progress, share best practices, and address any challenges.
Requirements
  • Current registration with the relevant professional body (RMN, OT, or Social Worker).
  • Significant post-registration experience, preferably in adult acute mental health work.
  • Proven experience of working with clients with complex needs, including those with moderate to severe mental health problems.
  • Excellent communication and interpersonal skills, with the ability to form productive working relationships with patients, carers, and multidisciplinary staff.
  • Comprehensive IT skills, with experience of using electronic patient records and other healthcare software.
  • Ability to work autonomously, prioritize tasks, and manage time effectively in a fast-paced environment.
Person Specification
  • Good standard of general education.
  • RMN, OT, or Social Worker qualification.
  • Current registration with the relevant professional body.
  • Significant post-registration experience.
  • Proven experience of working with clients with complex needs.
