About the Role

We are seeking an experienced and skilled nurse to join our Older Peoples Mental Health Intensive Home Treatment Team in Oldham. As a key member of our team, you will work closely with our Consultant Psychiatrist, community mental health team, care home liaison team, memory assessment team, and day hospital to provide high-quality care to individuals with complex mental health needs.

Key Responsibilities
  • Triage referrals and visit patients in their own homes or care homes to assess their mental health needs.
  • Complete comprehensive mental health assessments, risk assessments, and care plans to ensure individuals receive the best possible care.
  • Support students and TNA's in their learning and development.
  • Assess physical health and screen for any underlying medical conditions.
  • Utilize Trust-approved assessment tools to inform care decisions.
  • Discuss plans with the team and Consultant Psychiatrist to ensure seamless care coordination.
  • Administer and monitor medication in the community to ensure individuals receive the best possible treatment.
  • Prevent unnecessary admissions to wards by providing intensive support at home.
  • Gatekeep admissions to inpatient services and facilitate early discharge/leave pathways.
  • Attend ward rounds and discharge planning meetings to ensure continuity of care.
  • Safeguard individuals and their families by identifying and mitigating risks.
  • Provide education and support to patients and families in the community to promote recovery and well-being.
  • Work closely with the wider service, social care, and other agencies to ensure comprehensive care.
  • Record all activity on the PARIS system to ensure accurate and up-to-date records.
  • Liaise with GPs in the community and pharmacy to ensure seamless care coordination.
  • Direct the work of support workers in the team to ensure effective care delivery.
  • Engage in weekly zoning meetings with the team and managers to ensure effective communication and care coordination.
Requirements
  • Significant post-registration experience in mental health nursing.
  • Proven experience of working with clients with complex needs.
  • Experience of working with individuals who present to mental health services in crisis and have symptoms of mental health illness.
  • Experience of working in multi-disciplinary teams.
  • Experience of providing supervision.
Desirable Qualifications and Experience
  • Adult acute mental health work experience.
  • Experience of using outcome measures such as CORE, PHQ9, and GAD7.
  • Experience of working with a recovery model of care.
Knowledge and Skills
  • Understanding of current mental health legislation, safeguarding, and child protection legislation.
  • Awareness of NICE guidance in the treatment of mental health problems and knowledge of treatments and interventions appropriate for individuals with mental health needs.
  • Understanding of confidentiality and data protection issues.
  • Knowledge of psychosocial assessment and risk assessment/management processes.
