About the Role
The Hywel Dda University Health Board is seeking a highly skilled and experienced Diabetes Assistant Practitioner to join the Community Diabetes Team in Pembrokeshire. This is a unique opportunity to be part of an exciting new project aimed at improving access to diabetes surveillance, care, and education for those who are housebound or not able to access primary care interventions in our locality.
Main Responsibilities
- Work with the CNS team and primary and community colleagues to expedite proactive foot checks and foot health surveillance.
- Perform ankle-brachial pressure index (ABPI) testing, deliver smoking cessation advice, diabetic education, and liaise with diabetic podiatry, vascular, and retinopathy services.
- Monitor and record vital observations and interpret results, review risk assessments, and experience of supporting an RN with wound care.
Requirements
- Level 4 vocational qualification in a relevant health care associated subject/Certificate in Health Care Support Worker Education.
- GCSE Maths and English Grade C or above or equivalent level 2 or above essential skills qualifications.
- Evidence of continuing personal development relevant to the role in health care.
- Able to demonstrate understanding of the HCSW Code of Conduct, All Wales Delegation guidelines, and the All Wales Band 4 Assistant Practitioner Governance Framework.
Desirable Qualifications and Experience
- Local knowledge of statutory and voluntary services.
- Teaching or assessment qualification.
- Experience in a range of clinical skills including palliative care.
- Administration of medicines via oral, topical, SC, IM routes.
- Skills in non-complex wound care.
