Position Overview

We are pleased to announce an opportunity for an Advanced Practice Nurse to become a vital member of our Urgent Community Response and Virtual Ward Team.

We seek a proactive and motivated professional to assist in delivering the 2-hour Urgent Community Response and Virtual Ward services aimed at enhancing out-of-hospital care and facilitating the management of patients within their residences.

Our team provides immediate care to individuals in their homes, helping to prevent unnecessary hospital admissions and allowing patients to maintain their independence for a longer duration. We enable older adults and those with intricate health requirements to access a wide array of health and social care professionals within a two-hour timeframe.

This is an opportune moment to join us as we broaden our Virtual Ward/Hospital at Home initiative.

Virtual wards deliver hospital-level care and remote monitoring in the comfort of patients' homes, allowing them to receive care where they feel most at ease.

Emerging evidence suggests that patients experience improved outcomes when treated at home rather than in a hospital setting.

The role involves collaboration with a multidisciplinary team, including General Practitioners, Advanced Clinical Practitioners, Senior Nurses, Therapists, and Health Care Assistants, leveraging digital technology to enhance patient care.

Key Responsibilities

The Advanced Practice Nurse will be responsible for assessing, planning, implementing, and reviewing patient care in their homes, utilizing research and evidence-based knowledge to inform practice and support the avoidance of hospital admissions.

Responsibilities include:

  • Monitoring and enhancing care standards through practice supervision, clinical audits, evidence-based practices, and providing skilled clinical leadership.
  • Conducting assessments during crisis situations and delivering appropriate nursing interventions to meet patient needs.
  • Leading and guiding staff as necessary to meet service demands while effectively utilizing skills and resources.
  • Serving as an expert resource within the team for all aspects of nursing care.
  • Demonstrating advanced problem-solving abilities to facilitate patient-centered care and employing strong communication skills to achieve positive outcomes.
  • Participating in performance and development reviews within the team and overseeing the review of junior staff under managerial supervision.
  • Engaging in the investigation of complaints and safeguarding issues related to nursing services, promoting quality improvement initiatives.
