We are thrilled to present an opportunity for an Advanced Practice Nurse to become a vital member of our Urgent Community Response/Virtual Ward Team.

We seek a proactive and passionate healthcare professional to facilitate the implementation of the 2-hour Urgent Community Response and Virtual Ward initiatives aimed at enhancing out-of-hospital care and assisting patients in managing their health from the comfort of their homes.

This team is dedicated to providing immediate care to individuals in their residences, thereby preventing unnecessary hospital admissions and promoting independent living for a longer duration. Our services are particularly focused on older adults and individuals with intricate health requirements who urgently need assistance, ensuring they receive prompt access to a variety of healthcare and social services within a two-hour timeframe.

Joining us now presents an exciting opportunity as we expand our Virtual Ward/Hospital at Home services.

Virtual wards deliver hospital-level care and remote monitoring within patients' homes, allowing them to remain in a familiar and comfortable environment.

Research indicates that patients often experience improved outcomes when treated at home rather than in a hospital setting.

The successful candidate will collaborate with a multidisciplinary team that includes General Practitioners, Advanced Clinical Practitioners, Senior Nurses, Therapists, and Health Care Assistants, utilizing cutting-edge digital technology.

Key Responsibilities

The Advanced Practice Nurse will be responsible for assessing, planning, implementing, and reviewing patient care in their homes, employing evidence-based knowledge to support practices that prevent hospital admissions.

Additionally, the role involves:

  • Monitoring and enhancing care standards through supervision, clinical audits, and evidence-based practices.
  • Providing skilled clinical leadership and fostering learning and development within the team.

Job Responsibilities

The Advanced Practice Nurse will:

  • Conduct assessments for patients in crisis, providing appropriate nursing interventions and follow-ups.
  • Develop, implement, and review comprehensive nursing care plans based on research and evidence.
  • Lead and delegate tasks among team members according to service needs.
  • Serve as an expert resource within the team for all nursing care aspects.
  • Exhibit advanced problem-solving skills to facilitate patient-centered care.
  • Engage in the Performance and Development Review process and support junior staff development.
  • Participate in quality improvement initiatives.
