About the Role

We are seeking a highly motivated and enthusiastic Rehabilitation Support Worker to join our Extended Primary Care Team within Community Health services at East London NHS Foundation Trust.

Job Summary

The successful candidate will assist physiotherapists and occupational therapists in providing treatment to patients, carrying out rehabilitation programs to maximize patients' mobility and independence in daily activities in their own home and outdoors.

Main Responsibilities
  • Assist in the provision of rehabilitation programs to patients, as devised by physiotherapists and occupational therapists.
  • Work closely with multidisciplinary teams to provide comprehensive care to patients.
  • Develop and maintain effective relationships with patients, families, and healthcare professionals.
  • Contribute to the development and implementation of individualized care plans.
  • Participate in regular team meetings and contribute to the development of the team's goals and objectives.
Requirements
  • Experience working with older people or people with disabilities.
  • Experience of client-centered working, within a multidisciplinary team.
  • Ability to manage own workload, work flexibly, and use initiative to problem-solve.
  • Organizational, planning, and time management skills.
  • Good interpersonal and written communication skills.
  • Ability to work as part of a multi-disciplinary team.
  • Ability to recognize boundaries of own skill level and seek help as appropriate.
  • Ability to establish a therapeutic relationship with the client.
  • Ability to assist clients who are frail or who require assistance with activities of daily living and to apply a rehabilitation approach in a support worker role.
  • Word-processing/IT skills.
