About the Role

We are seeking a dedicated and compassionate Health Care Support Worker to join our team at Southern Health NHS Foundation Trust. As a Health Care Support Worker, you will play a vital role in delivering excellent care to our service users, working closely with the team to provide the right support and assistance to help them achieve their recovery goals.

Main Responsibilities

  • Provide physical and psychological care to service users in accordance with their individual care plans and the recovery model
  • Carry out duties as designated by the team leader
  • Support the planning and delivery of interventions in line with agreed care plans
  • Deliver higher level intensity community interventions as part of the integrated Shared Care model
  • Work in a manner that is helpful to the service user and the referrer, based on principles of recovery, personalisation and choice
  • Work with individuals in the least restrictive manner possible
  • Have previous experience in working with people with mental health needs and be able to evidence effective team working
  • Complete phlebotomy training to take bloods for our clozapine clinic
  • Have access to a vehicle for work purposes
