About the Role
Southern Health NHS Foundation Trust is seeking a Health Care Support Worker to join their Community Mental Health Team at Avalon House in Winchester. As a key member of the team, you will play a vital role in delivering excellent care to our service users, working collaboratively with colleagues to provide the right support and assistance to those in need.
The successful candidate will have previous experience in working with individuals with mental health needs, with a strong focus on recovery and personalization. You will be able to evidence effective team working and have the ability to drive and use a car for work purposes.
Main Responsibilities
- Provide physical and psychological care to service users in accordance with the specified plan of clinical care.
- Carry out duties as designated by the team leader.
- Support the planning and delivery of interventions in line with an agreed care plan.
- Deliver higher level intensity community interventions as part of the integrated Shared Care model.
- Work in a manner which is helpful to the service user and the referrer, based on principles of recovery, personalization and choice.
- Work with individuals in the least restrictive manner possible.
