About the Role

We are seeking a highly motivated and compassionate Health Care Support Worker to join our Community Mental Health Team at Avalon House in Winchester. As a key member of our team, you will play a vital role in delivering excellent care to our service users, working closely with individuals to promote their recovery and well-being.

Key Responsibilities
  • Provide physical and psychological care to service users in accordance with their individual care plans, promoting a recovery-focused approach.
  • Support the planning and delivery of interventions, working collaboratively with the multidisciplinary team to achieve positive outcomes.
  • Deliver higher-level intensity community interventions as part of our integrated Shared Care model, ensuring seamless care coordination.
  • Work in a manner that is helpful to the service user and referrer, based on principles of recovery, personalization, and choice.
  • Support the person responsible for the clinical care of the service user, ensuring accurate recording of service user information.
  • Provide support to relatives and carers, promoting a holistic approach to care.
