Position Overview

As a Senior Nurse Practitioner in a primary care environment, the individual will make independent, professional decisions for which they will hold full accountability.
The Senior Nurse Practitioner will address patients with undifferentiated and undiagnosed health issues. Key responsibilities will encompass assessment and treatment, including prescribing when appropriate, screening, and patient education.
The primary objective of this role is to ensure patients have direct access to primary care services, while fostering a skilled and balanced clinical team to enhance overall patient care.
Key Responsibilities
Clinical Duties:
Serve as the initial point of contact for patients seeking primary care for designated conditions, conducting thorough health assessments based on advanced nursing expertise. Provide guidance, initiate treatment, and refer to the on-call GP as necessary.
Prescribe medications tailored to the needs of the practice population.
Manage minor illness clinics to facilitate early diagnosis and refer patients as needed for further intervention.
Collaborate with other primary, community, and secondary care professionals within established local pathways.
Partner with general practitioners, practice nurses, and other professionals to develop protocols, standards, and audit tools to ensure effective care delivery.
Identify, assess, and diagnose patients with complex health needs across the practice population.
Lead the introduction and application of specialized nursing treatments for patients with intricate healthcare requirements.
Develop and implement programs aimed at improving health and well-being, addressing the specific needs of communities, groups, and individuals.
Collaborate with others to influence policies that enhance health and well-being and reduce disparities.
Drive the implementation and evaluation of evidence-based practices to enhance the services provided by the nursing team.
Work alongside other team members to implement specific aspects of national service frameworks.
Chronic Disease Management
Oversee diagnosis, monitoring, and the development of individualized management plans, collaborating with patients and other health professionals as appropriate.
Therapeutic Monitoring
Assess compliance with and adherence to prescribed treatments, employing a holistic, patient-centered approach.
Recognize abnormalities and identify the effects of treatment, making recommendations for adjustments as necessary.
Risk Assessment
Identify potential issues and gather sufficient information for referrals (e.g., substance abuse, domestic violence, child protection, vulnerable adults, elderly patients, social challenges).
Health Screening
Provide comprehensive monitoring and advice as appropriate, collaborating with patients to determine management plans.
Consider the needs of the practice population in decision-making.
First Contact Care
Function as an autonomous practitioner for more complex cases.
Ear Care
Address more intricate issues and prescribe as necessary.
Mental Health
Ensure effective patient monitoring systems are in place.
Administrative and Professional Duties
Engage in the administrative and professional responsibilities of the practice team.
Participate in policy development for clinical matters, providing nursing insights into decision-making processes affecting patient care.
Foster multi-disciplinary and multi-agency collaboration in the development of practice initiatives.
Ensure the collection and maintenance of statistical data required for regular and ad hoc reporting.
Education, Training, and Professional Development
Pursue ongoing education through relevant courses and study days, maintaining a personal learning plan aligned with revalidation requirements.
Contribute to the education of primary and community care clinicians across all disciplines, with a focus on nursing staff.
Create a learning environment that encourages lifelong learning.
Participate in research projects within the practice as required.
Health Promotion and Disease Prevention
Target health promotion initiatives based on the epidemiological and health needs of the practice population, as outlined in the Health Improvement Programme.
Assist in organizing health promotion and chronic disease management programs within the practice.
Manage and audit national and local screening programs for the practice population.
Special Requirements
- Registered General Nurse
- First level degree or equivalent
- Compliance with NMC Code of Conduct
- Independent Prescriber V300 qualification
- Minimum of three years' experience in community nursing (District Nursing, Health Visiting, or Practice Nursing)
- Strong self-motivation, organizational, and prioritization skills
- Excellent communication abilities
- In-depth knowledge of current clinical practices
- Understanding of clinical governance
- Awareness of contemporary professional issues in nursing
