Service Care Solutions is seeking a dedicated Community Support Worker to join our team in Cheshire East. As a Community Support Worker, you will play a vital role in providing high-quality care and support to service users, promoting their independence and well-being.
**Key Responsibilities:**
* Attend to the personal needs of service users, undertaking scheduled checks in line with agreed care programmes.
* Contribute to the assessment of service users by carrying out assessment tasks and maintaining and updating records as required.
* Participate in discussions on Individual Care Programmes for service users and act as a key worker when required.
* Implement initial actions in situations likely to threaten the safety or security of service users, reporting to the Senior Community Support Worker.
