Job Summary:
We are seeking an experienced Community Mental Health Nurse to join our Assessment & Liaison Team on a locum basis. As a key member of our team, you will be responsible for providing high-quality patient care and contributing to the development of our services.
Key Responsibilities:
- Provide comprehensive mental health assessments and interventions to patients in the community
- Develop and implement individualized care plans in collaboration with patients, families, and other healthcare professionals
- Conduct regular reviews and evaluations to ensure optimal patient outcomes
- Collaborate with other healthcare professionals to ensure seamless care coordination
- Participate in quality improvement initiatives to enhance patient care and services
Requirements:
- Degree or equivalent in Mental Health Nursing
- Experience working as a Mental Health Nurse
- NMC Registration
