Our approach emphasizes the importance of home-based care and assessing needs post-discharge, ensuring clients can maintain their independence in a safe environment. We work closely with our reablement teams and rehabilitation services to enhance clients' autonomy and foster positive outcomes. When necessary, we engage with local Integrated Commissioning Boards to secure access to ongoing care funding for those we serve.
To excel in this role, you should possess a comprehensive understanding of the Mental Capacity Act (2005), the Mental Health Act (1983), equality legislation, and the Deprivation of Liberty safeguards. Implementing this legislation in your daily responsibilities is essential. You will also be responsible for fulfilling the County's safeguarding duties to protect clients and mitigate risks. Assessing clients in accordance with statutory obligations, such as the Care Act (2014), using a strengths-based and outcome-focused methodology is expected. Experience with court of protection matters is advantageous, though training will be provided to successful candidates.
Candidates must be Registered Social Workers with Social Work England and hold a recognized social work qualification, such as a C.Q.S.W, DipSW, or a Degree/Master's in Social Work. We encourage applications from Social Workers who have completed their Assessed and Supported Year in Employment.
