About the Role
We are seeking a highly skilled Mental Health Practitioner to join our team at Avon & Wiltshire Mental Health Partnership NHS Trust. As a Mental Health Practitioner, you will play a key role in delivering high-quality mental health services to our patients in Primary Care settings.
Key Responsibilities
- Contribute to the development and delivery of mental health services in Primary Care, working closely with GPs, social prescribers, and other healthcare professionals.
- Assess and advise on the mental health needs of patients, providing evidence-based interventions and support.
- Work collaboratively with colleagues in specialist community mental health services to ensure seamless transitions between teams and services.
- Connect patients with community and voluntary sector support, promoting a strengths-based approach to mental health care.
- Facilitate mental health and strengths-based assessments in Primary Care, supporting the Primary Care Team in accessing appropriate levels of services.
- Build and maintain strong relationships with Primary Care Networks, Community Mental Health Teams, Social Care teams, and voluntary sector organizations.
- Provide targeted learning opportunities for Primary Care staff to develop their knowledge and capabilities in mental health care.
- Provide information, signposting, and support for patients and their relatives/carers with mental health needs.
- Foster collaborative and trusting working relationships with referring agents and Primary Care Health Professionals.
Requirements
- Professional qualification in mental health (e.g., Registered RMN, Registered Social Worker, or Registered Therapist).
- Current registration with a professional body and commitment to Continuing Professional Development (CPD).
- Significant experience in educating and training the mainstream workforce, particularly non-specialist mental health workers.
- Demonstrable experience of assessing risk and developing risk management strategies.
