About the Role

We are seeking a highly skilled Mental Health Practitioner to join our team at Avon & Wiltshire Mental Health Partnership NHS Trust. As a Mental Health Practitioner, you will play a key role in delivering high-quality mental health services to our patients in Primary Care settings.

Key Responsibilities

  • Contribute to the development and delivery of mental health services in Primary Care, working closely with GPs, social prescribers, and other healthcare professionals.
  • Assess and advise on the mental health needs of patients, providing evidence-based interventions and support.
  • Work collaboratively with colleagues in specialist community mental health services to ensure seamless transitions between teams and services.
  • Connect patients with community and voluntary sector support, promoting a strengths-based approach to mental health care.
  • Facilitate mental health and strengths-based assessments in Primary Care, supporting the Primary Care Team in accessing appropriate levels of services.
  • Build and maintain strong relationships with Primary Care Networks, Community Mental Health Teams, Social Care teams, and voluntary sector organizations.
  • Provide targeted learning opportunities for Primary Care staff to develop their knowledge and capabilities in mental health care.
  • Provide information, signposting, and support for patients and their relatives/carers with mental health needs.
  • Foster collaborative and trusting working relationships with referring agents and Primary Care Health Professionals.

Requirements

  • Professional qualification in mental health (e.g., Registered RMN, Registered Social Worker, or Registered Therapist).
  • Current registration with a professional body and commitment to Continuing Professional Development (CPD).
  • Significant experience in educating and training the mainstream workforce, particularly non-specialist mental health workers.
  • Demonstrable experience of assessing risk and developing risk management strategies.
