Mental Health Practitioner

We are seeking a skilled Mental Health Practitioner to join our Primary Mental Health Team in North Cornwall. As a key member of our team, you will work closely with GPs, practice nurses, and other healthcare professionals to provide high-quality mental health care to our patients.
Main Responsibilities- Deliver timely and expert assessment to improve access to care options.
- Implement evidence-informed approaches to address both mental and physical health.
- Reduce referrals to specialist services by providing earlier interventions.
In partnership with the Primary Care Network and Cornwall Foundation Trust, we aim to deliver joined-up, patient-centered, trauma-informed, and place-based care that optimizes community assets.
Key Skills and Qualities- Effective risk management skills.
- Experience and awareness of common acute and long-term conditions in general practice.
We are looking for a motivated and experienced professional who can work collaboratively with our team to deliver high-quality care to our patients.
