Position Overview

We are seeking dedicated Care Coordinators to enhance our current team in delivering exceptional Community Care Services to our clients.

Key Responsibilities:

  • Developing comprehensive work schedules and individual shift plans for the RSD Care Workers.
  • Creating efficient Care rotas that meet the needs of Service Users while optimizing the workflow of RSD Care Workers.
  • Collaborating with multidisciplinary teams from Local Authorities and Health Boards.
  • Adjusting shift schedules in response to worker absences.
  • Providing personal care during urgent situations.
  • Delivering clear written and verbal instructions to RSD Care Workers.
  • Overseeing the Emergency On Call service during evenings and weekends (on a rotational basis).

Required Skills:

Candidates must be willing to work outside of standard office hours and on weekends.

  • Possession of a valid driving license and access to a vehicle (Business insurance is necessary).
  • Strong telephone etiquette and communication skills, with the ability to effectively manage staff and foster a positive work environment.
  • Preferably holding NVQ level 2/3 in Care (Training will be provided by RSD).
  • Excellent organizational and problem-solving abilities.
  • Proficient in computer usage with good literacy and numeracy skills.

Additional Requirement:

Must have a valid Driver's License and personal transportation.
