About the Role

We are seeking a highly motivated and experienced Registered Nurse to join our dynamic team as a Deputy Team Lead in the Hertfordshire Division of the Central London Community Health Trust (CLCH). This is an exciting opportunity to lead one of our neighbourhood teams under the direction and leadership of the Deputy Locality Manager.

Key Responsibilities
  • Manage a patient caseload, assessing, planning, implementing, and evaluating evidence-based care to patients in their home environment.
  • Develop and implement new community pathways to support the delivery of more care in people's homes.
  • Collaborate with the team to develop and implement new learning and development opportunities through the CLCH Academy.
  • Provide leadership and guidance to the team, promoting a culture of excellence and continuous improvement.
  • Manage change within a complex environment, ensuring seamless transitions and minimal disruption to services.
Requirements
  • Registered Nurse with current UK NMC registration.
  • Mentor/Preceptor/998 or equivalent.
  • Specialist Nursing Practice (District Nursing) Degree or will undertake training.
  • Nurse Prescribing or will undertake training.
  • Evidence of personal development.
  • Minimum of 3 years' experience at band 5 Community Nurse level.
  • Management of a team.
  • Management of change within a complex environment.
  • Experience of teaching others.
  • Proven leadership ability.
  • Proven clinical skill at Senior Community Nursing level.
Desirable Qualifications and Experience
  • First Level Degree.
  • Independent Prescriber.
  • Relevant courses in Long Term Conditions Management.
  • Course of Top to Toe examination/diagnosis experience.
