Job Summary

CLCH Hertfordshire Adult Community Services, part of CLCH NHS Trust, is seeking a highly motivated and experienced Registered Nurse to fill the role of Deputy Team Lead. This challenging and rewarding position offers the opportunity to lead one of our neighbourhood teams, working closely with the Deputy Locality Manager.

About the Role

As a Deputy Team Lead, you will be responsible for a patient caseload, assessing, planning, implementing, and evaluating evidence-based care to patients in their home environment. Your experience in community care, leadership skills, and ability to work autonomously will be essential in this role.

Key Responsibilities
  • Lead a neighbourhood team under the direction of the Deputy Locality Manager
  • Assess, plan, and implement evidence-based care to patients in their home environment
  • Work collaboratively with other healthcare professionals to deliver high-quality patient care
  • Provide leadership and guidance to junior staff members
  • Participate in clinical governance and contribute to the development of policies and procedures
Requirements
  • Registered Nurse with current UK NMC registration
  • Mentor/Preceptor/998 or equivalent
  • Specialist Nursing Practice (District Nursing) Degree or will undertake training
  • Nurse Prescribing or will undertake training
  • Experience of management and leadership in a community care setting
