Job Summary

Central London Community Health Trust is seeking a highly skilled and experienced Registered Nurse to join our Community Nursing team as a Deputy Team Lead. This is an exciting opportunity to work in a dynamic and forward-thinking team, providing high-quality care to patients in their homes.

About the Role

As a Deputy Team Lead, you will be responsible for leading a team of nurses and providing clinical guidance and support to ensure the delivery of excellent patient care. You will work closely with the Deputy Locality Manager to develop and implement care plans, and will be responsible for managing a patient caseload, including assessment, planning, implementation, and evaluation of evidence-based care.

Key Responsibilities
  • Lead a team of nurses and provide clinical guidance and support
  • Develop and implement care plans in collaboration with the Deputy Locality Manager
  • Manage a patient caseload, including assessment, planning, implementation, and evaluation of evidence-based care
  • Work closely with other healthcare professionals to ensure seamless care delivery
  • Participate in clinical audits and quality improvement initiatives
Requirements

To be successful in this role, you will need:

  • Current UK NMC registration as a Registered Nurse
  • Experience of working in a community nursing setting
  • Strong leadership and management skills
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Knowledge of current clinical governance and evidence-based practice
