We are currently seeking dedicated Rehabilitation Support Specialists to become integral members of our diverse teams, which operate in various environments, including hospital and community rehabilitation services.
These roles are dynamic and require a collaborative approach to enhance patient flow, facilitate timely discharges, and support long-term adaptation and reintegration into the community. Our primary objective is to elevate the health and wellbeing of the populations we serve.
We are in search of highly motivated, proactive individuals who excel in teamwork and possess outstanding organizational and interpersonal skills. The ability to prioritize tasks effectively and communicate proficiently is crucial. A methodical and organized work style, along with the capacity to work independently once fully trained, is essential. The specifics of the service will be guided by local service requirements.
Flexibility is key, as you will be expected to adapt to current service demands across our 7-day operational framework. Community-based roles necessitate regular driving and may involve working alone in accordance with service policies.
Key Responsibilities
- Engage with individuals and caregivers who may struggle to access services, managing any uncooperative or challenging behaviors that arise.
- Utilize your skills and knowledge to identify significant changes in the holistic needs of service users and provide feedback to registered practitioners.
- Maintain accurate and up-to-date records as mandated by the Partnership Trust and in compliance with HCPC standards.
- Assist in creating therapeutic resources under the guidance of registered practitioners as needed.
- Employ problem-solving and innovative strategies to deliver high-quality interventions while addressing unplanned demands of the role.
- Participate in regular supervision sessions.
- Contribute to the onboarding process for new team members by demonstrating duties as required.
- Identify personal training and educational needs during supervision and appraisal processes in collaboration with registered practitioners, ensuring ongoing professional development.
- Collect and input statistical data as required.
- Support the team in conducting clinical audits.
- Actively promote adherence to policies, procedures, guidelines, and relevant statutory measures.

- Relevant experience in roles such as: teamwork, care environments, supporting individuals with disabilities, implementing rehabilitation programs, and facilitating group activities.
Desirable
- Experience in assisting service users, families, and caregivers in managing the effects of compromised functional abilities.
Qualifications
Essential
- QCF/NVQ Level 3 or equivalent in a related field, or evidence of comparable academic achievement.
- Willingness to undertake relevant training for the position.
Desirable
- Completion of the Care Certificate across all 15 areas within 12 weeks (pro rata) of starting the role.
- Relevant experience in supporting individuals with compromised functional abilities.
