About the Role

We are seeking a highly skilled and dedicated Social Worker/AMHP to join our Community Mental Health Team. As a key member of the team, you will be responsible for providing high-quality social work interventions to individuals with complex mental health needs.

Key Responsibilities
  • Conduct comprehensive assessments and develop effective care plans to support service users in achieving their recovery goals.
  • Work collaboratively with multidisciplinary teams to provide holistic care and support to individuals with severe and enduring mental health needs.
  • Champion the Social Model within secondary mental health services, promoting a person-centred approach to care and support.
  • Contribute to the development and implementation of policies and procedures that promote best practice in social work and mental health care.
Requirements

To be successful in this role, you will possess:

  • Excellent communication and interpersonal skills, with the ability to work effectively with individuals, families, and multidisciplinary teams.
  • Strong assessment and intervention skills, with the ability to develop and implement effective care plans.
  • Knowledge of mental health legislation and policy, with the ability to apply this knowledge in practice.
  • A commitment to the Social Model and a person-centred approach to care and support.
