We are seeking an experienced and skilled Advanced Practitioner to join our team at Central and North West London NHS Foundation Trust. The successful candidate will have a strong background in nursing and a proven track record of leadership and clinical expertise. You will be responsible for prioritizing health problems, intervening in complex situations, and leading resuscitation efforts. Additionally, you will be expected to work autonomously, within ethical codes and legal frameworks, and be accountable for your decisions and actions.
Key Responsibilities- Deliver high-quality patient-centered care, promoting clinical excellence and providing clinical advice and support to nursing and healthcare professionals.
- Prioritize health problems, intervene in complex situations, and lead resuscitation efforts.
- Work autonomously, within ethical codes and legal frameworks, and be accountable for your decisions and actions.
- Advocate high-quality care for people, act as a champion for people, and represent/support the person's interests at team meetings, during hospital care, home care, and when interfacing with statutory and non-statutory service providers.
- Influence, support, and take an active role in Public Health/Self-care of the individual and community.
- Collect, collate, evaluate, and report clinical information, maintain accurate patient records related to assessment and care planning.
- Develop clinical protocols and guidelines within your area of practice to ensure quality of care at all times.
- Current registration with the Nursing and Midwifery Council (NMC).
- Relevant professional health degree.
- Additional leadership training.
- Appropriate therapy training/qualification.
- Leadership experience at Band 7 or above.
- In-depth knowledge of recent NHS legislation, relevant to community.
- Experience of leading, planning, and completing audit/research.
- Knowledge of health and social care legislation relevant to health for adults in Prison/Detention Centre Health.
- Knowledge of key framework and policy practice issues relevant to Prison/Detention Centre health.
- Management skills with team-building abilities.
- Ability to work alone as well as be part of a team.
- Ability to implement policy and guidelines.
