About the Role

We are seeking a highly skilled and dedicated District Nurse to join our community nursing team in Woking. As a key member of our team, you will be responsible for delivering high-quality patient care and leading a dynamic team of healthcare professionals.

Key Responsibilities:

  • Deliver and maintain a high standard of clinical and professional nursing care
  • Assist in the delivery of all aspects of individualized patient care
  • Lead and shape a dynamic team of healthcare professionals
  • Develop and implement effective care plans in collaboration with patients, families, and healthcare teams
  • Monitor and evaluate patient outcomes, making adjustments to care plans as needed
  • Collaborate with other healthcare professionals to ensure seamless patient care
  • Supervise and mentor junior staff members, providing guidance and support as needed
  • Participate in clinical audits and quality improvement initiatives to enhance patient care and outcomes

Requirements:

  • Current registration as a Registered Nurse (RGN/RMN/RPN)
  • Degree in a relevant healthcare field (or working towards)
  • Evidence of continued professional development post-registration
  • Clinical assessment and supervision skills
  • Ability to work autonomously and as part of a team
  • Excellent communication and interpersonal skills
  • Ability to prioritize tasks and manage time effectively
