Job Summary

As a key member of the Ashford and St. Peter's Hospitals NHS Foundation Trust team, you will be responsible for managing and leading the outreach anticoagulation clinics at Woking Community Diagnostic Hub. Your primary goal will be to implement effective treatment plans for patients requiring anticoagulation therapy, while also contributing to the organization and day-to-day running of the Anticoagulant Service across Woking and surrounding areas.

Main Responsibilities

Key responsibilities will include:

  • Managing and organizing anticoagulant outreach clinics at Woking Community Diagnostic Hub, including liaison with GP, SPA, and District Nurse Teams
  • Providing initial and ongoing education to patients, carers, and healthcare professionals on anticoagulation therapy and management
  • Collaborating with the multidisciplinary team to develop and implement effective treatment plans for patients requiring anticoagulation therapy
  • Maintaining accurate and up-to-date computerized records using DAWN and Cerner software
