Your Role

As a Primary Care Mental Health Practitioner, you will play a vital role in delivering responsive mental health interventions within the community. You will act as a bridge between primary and secondary mental health services, collaborating with patients, families, and other healthcare professionals to ensure comprehensive care.

Key Responsibilities

  • Conduct thorough holistic assessments of patients, supporting shared decision-making and self-management strategies.
  • Provide evidence-based brief biopsychosocial interventions tailored to individual needs.
  • Demonstrate a strong understanding of health inequalities and implement strategies to ensure accessibility for all.
  • Network with community resources and providers to facilitate access to appropriate mental healthcare options.
  • Collaborate effectively with patients, carers, families, and the wider multi-agency team.
