About the Role

We are seeking an experienced Advanced Care Practitioner, Prescribing Nurse, or Paramedic to join our Primary Care Network team as a Home Visiting Service Lead. This is an exciting opportunity to work in a dynamic and innovative environment, providing high-quality patient care and contributing to the development of our Home Visiting Service.

Key Responsibilities
  • Provide clinical leadership and guidance to the Home Visiting Service team, ensuring the delivery of high-quality patient care and adherence to best practice.
  • Develop and implement effective care pathways and protocols to improve patient outcomes and reduce hospital admissions.
  • Work closely with the Frailty Team to provide comprehensive care to patients with complex needs, including those with frailty, chronic disease, and mental health conditions.
  • Collaborate with other healthcare professionals, including GPs, nurses, and therapists, to ensure seamless care coordination and patient-centered care.
  • Contribute to the development and implementation of new services and initiatives to improve patient care and outcomes.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures to ensure best practice.
Requirements
  • Current registration with the relevant professional body (NMC or HCPC).
  • Experience in a similar role, with a strong track record of delivering high-quality patient care and improving patient outcomes.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work autonomously and as part of a team, with a strong commitment to patient-centered care.
  • Knowledge of relevant policies, procedures, and guidelines, including those related to frailty, chronic disease, and mental health.
