Job Description

Job Title: Senior End of Life Care Specialist

Job Summary:

The University Hospitals Plymouth NHS Trust is seeking a highly skilled and experienced Senior End of Life Care Specialist to lead and develop our End of Life Care services. As a key member of our clinical leadership team, you will be responsible for designing, coordinating, and implementing evidence-based service delivery across our End of Life Care teams.

Main Responsibilities:

  • Develop and maintain clinical excellence in End of Life Care services, ensuring high-quality patient care and outcomes.
  • Provide expert consultancy services to colleagues, including pathways into and out of the Emergency Department, to support remote advice and support.
  • Plan, implement, and evaluate evidence-based service delivery across areas of responsibility, ensuring alignment with Trust initiatives and End of Life transformation plans.
  • Support the development of the service by generating and disseminating knowledge, providing education across the organization, and contributing to the development of the service.
  • Ensure the service to patients, families, and carers focuses on improving outcomes and experiences for End of Life patients, families, and carers.

Key Requirements:

  • In-depth knowledge and experience working with processes, services, and pathways that support admission avoidance and enable preferred place of care/death.
  • The ability to practice autonomously using specialist skills of assessment, investigation, and management in End of Life patient care.
  • Non-medical prescribing qualification or actively working towards.

Desirable Qualifications and Experience:

  • Leadership experience within the acute setting and managing teams.
  • Experience of community End of Life care and support.
  • Masters degree in a related subject.

Advanced Communication Skills Training and Advanced Communication 'Train the Trainer' qualification are also desirable.
