End of Life Care Specialist

We are seeking a skilled Registered Nurse to join our team in providing high-quality, person-centred care to patients with terminal illnesses and their families. The successful candidate will assume responsibility for assessing, planning, implementing, and evaluating evidence-based care, while delegating tasks to level 2-4 practitioners as necessary.
Main Responsibilities- Deliver hands-on care and emotional support to patients and their families in the Emergency Department.
- Identify and assist patients nearing the end of life within the acute hospital, ensuring timely transfers to their preferred place of care.
- Provide specialised care within Mount Gould Community Hospital.
- Work with a dedicated team of volunteers to provide companionship and advance care planning.
- Registered Nurse on the NMC Register (Adult).
- Ability to demonstrate continued professional development.
- Relevant post-registration training, such as palliative and end-of-life care, leadership, or quality improvement.
