Job Summary

This position requires a skilled professional to work with clients, connecting them with supportive community engagement and employment services. The incumbent will provide services in the client's home and in the community to support integration and stabilization. All services are provided as part of a comprehensive treatment team, including a doctor, nurses, therapists, and case managers.

Key Responsibilities
  • Provide vocational rehabilitation services to clients, including job coaching and placement support.
  • Work with clients to develop and implement individualized service plans, focusing on community integration and stabilization.
  • Collaborate with the treatment team to ensure comprehensive support for clients.
  • May involve weekend duties to maximize community integration for clients, as well as occasional scheduled duties beyond traditional work hours.
Qualifications
  • Master's degree in a relevant field, such as social work, counseling, or psychology.
  • Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW) eligible, with licensure preferred.
  • Professional and clinical experience in vocational rehabilitation, with a focus on disability support services.
Additional Requirements
  • Valid Driver's License and good driving record, with the ability to provide own transportation.
  • Ability to work in a variety of environments, including client homes and community settings.
  • Exposure to allergenic environments, including extreme heat, humidity, dust, smoke, pet contamination, and cleaning supplies.
  • Exposure to clients with infectious diseases.
  • Ability to drive for long periods of time to visit clients in and out of the catchment area as needed.
  • Ability to walk distances, climb and descend stairs, and maneuver in and out of agency vehicles.
  • Prolonged periods sitting at a desk and working on a computer.
  • Ability to lift/push/pull up to 25 pounds at times.
