Job Summary

We are seeking a highly motivated and organized Care Coordinator to join our Primary Care Network team. As a Care Coordinator, you will play a vital role in supporting the delivery of local enhanced services and other PCN projects that improve health outcomes for our patients.

Key Responsibilities
  • Support the delivery of PCN Enhanced Services across the network, including Diabetes and Mental Health.
  • Support achievement against QOF indicators, including childhood immunizations, cervical screening, Asthma, COPD, and more.
  • Respond to patient requests for each network practice by telephone, email, or SMS.
  • Support full utilization of practice appointments and PCN Enhanced Access appointments.
  • Provide administrative support for PCN projects and other ad-hoc work that enhances the resilience and sustainability of our network practices.
Requirements
  • Must have experience working in Primary care.
  • Experience using EMIS clinical system.
  • Experience with using computers.
Desirable Qualifications
  • Experience using ACCRUX.
  • Experience working in a GP practice.
