Care Coordinator at a Primary Care Network

We are seeking a highly skilled and motivated PCN Care Coordinator to join our team at East End Health Network PCN. As a PCN Care Coordinator, you will play a vital role in coordinating care packages for patients across health, social care, and mental health settings.
Main Responsibilities- Coordinate patient-centred care plans, monitoring progress and reporting outcomes, and contributing to patient reviews and care planning within appropriate time frames.
- Explain patient pathways to clinical staff, liaising between services and service users, and contacting services using the appropriate procedures/referral mechanisms.
- Work closely with care agencies to ensure coordinated delivery of patient care plans, without requiring further referrals from GPs.
- Maintain accurate records and statistical returns as required by the CCG, including providing patient-related information for Clinical Reporting Systems.
- Ensure proper handover of care between settings, including mutual transfer of communications and patient notes, and setting up care packages.
- Collect data on patients/carers for recognized outcome measures and document for service interpretation.
- Manage operational meeting processes, identifying patients for discussion, and working closely with clinicians to define and lead meetings.
- Ensure meeting actions are recorded, disseminated, and followed up in a timely way, and relevant practitioners are aware of meeting decisions and actions/outcomes.
- Network and develop strong relationships with NHS key players, including the CCG, GPs, and other primary care contractors, Social Services, Mental Health Trusts, and other providers.
- Relevant degree or equivalent level of training and experience.
- Evidence of consistent pattern of learning from education, training, and experience.
- Qualification in health or social care allied profession.
- Coordination of services from various organizations.
- Communication and relationship building.
- Experience in health or social care.
- Experience of developing relationships with a wide variety of people.
- Experience of health and social care assessments.
- Demonstrated successful outcomes delivery within determined timeframes.
