About the Role

We are seeking a highly skilled and motivated Mental Health Nurse Practitioner to join our Integrated Mental Health team in Lichfield. As a key member of our team, you will play a vital role in delivering high-quality care to individuals with mental health needs in the community.

Main Responsibilities
  • Work closely with colleagues to deliver quality and physical health quality improvement initiatives.
  • Ensure high-quality care for service users within the community, prioritizing physical health needs and promoting engagement with screening, assessment, and monitoring of physical health.
  • Develop and deliver agreed clinical processes, working collaboratively with the clinical team to assess, review, and monitor people's health, including reviewing prescribed medications as an NMP.
