Main Responsibilities- Work closely with colleagues to deliver quality and physical health quality improvement initiatives.
- Ensure high-quality care for service users within the community, prioritizing physical health needs and promoting engagement with screening, assessment, and monitoring of physical health.
- Develop and deliver agreed clinical processes, working collaboratively with the clinical team to assess, review, and monitor people's health, including reviewing prescribed medications as an NMP.
