Job Summary:

Kirklees Council is seeking experienced Social Workers, Senior Practitioners, and Mental Health Professionals to join our team. As a key member of our multidisciplinary team, you will be responsible for providing high-quality, strengths-based, and recovery-focused packages of care and support to vulnerable individuals and their families.

Key Responsibilities:

  • Conduct holistic strength-based assessments and commission personalized support plans for service users.
  • Provide at least restrictive and preventative approaches to service users to avoid hospital admission and ensure people remain in their homes.
  • Undertake Mental Health Act assessments with service users within the community as an Approved Mental Health Professional.
  • Work collaboratively with a multi-disciplinary team to provide a dynamic, responsive, and innovative service.

Requirements:

  • Working knowledge of the Care Act, Mental Health Act, Human Rights Act, SEND legislation, Mental Capacity Act, and associated Codes of Practice.
  • Experience of working with the service user group and using social work methods to promote positive change, independence, and wellbeing.
  • Social Work England recognized qualification and registration with Social Work England, subject to a DBS check.
