Patient Care Coordinator

Job Summary:
The Lewisham Care Partnership is seeking a highly skilled and compassionate Patient Care Navigator to join our team. As the first point of contact for patients, you will provide exceptional customer service, arrange appointments, and offer guidance on available services.
This role requires strong communication skills, the ability to work in a fast-paced environment, and a passion for delivering high-quality patient care.
Main Responsibilities:- Process and signpost patients to the appropriate healthcare professional based on their presenting condition.
- Answer incoming phone calls, transfer calls, or handle caller requests appropriately.
- Process patient requests for appointments and repeat prescriptions.
- Initiate contact with patients, team members, and external agencies as needed.
- Enter read-code data on the Emis Web clinical system and maintain accurate patient records.
- Perform administrative tasks, such as photocopying and data entry, as required.
- Support clinical staff with general tasks and maintain a clean and organized working area.
