About the Role

We are seeking a highly skilled and experienced Care Pathway Coordinator to join our team at Barking, Havering and Redbridge University Hospitals NHS Trust. As a key member of our Geriatric Division, you will play a vital role in ensuring the smooth discharge of patients from our Frailty Unit.

Key Responsibilities
  • Collaborate with the multidisciplinary team to coordinate discharge plans and ensure timely and safe patient transfers.
  • Work closely with nursing staff, medical teams, and other healthcare professionals to identify and address any barriers to discharge.
  • Develop and maintain effective relationships with external agencies and stakeholders to ensure seamless patient care.
  • Provide education and support to patients, families, and carers to promote self-management and independence.
  • Contribute to the development and implementation of care pathways and protocols to improve patient outcomes.
Requirements
  • Proven experience in an acute hospital clinical role, preferably in elderly care.
  • Excellent interpersonal and communication skills, with the ability to work effectively with diverse stakeholders.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple projects.
  • Ability to work autonomously and as part of a team, with a flexible approach to working hours.
  • Knowledge of patient management systems and care pathways, preferably in a hospital setting.
